Thanks Jeremy,
Having done further digging since I posted, I have found that the
problem regarding my httpd.conf setup which was NOT properly loading
mod_rewrite.so. The book didn't mention this requisite. :(
The file configuration I had setup works fine as well as does the one
you suggested.
