Because we can't see what what you are doing, some guesses ;

in particular item 2
"Include a copy of the "einsert.js" file"
and certainly item 3
"var insert = new EInsert(... "
and especially item 3.1
"The parameters are: A GLatLng() ... "
and nowhere is mentioned any .groundOverlay() method for EInsert
