[Google Maps API v2] opening google map link in android or iphone native app

i have a mobile website with links to google maps.

i want to open these maps not in the web browser, but in the native
google maps app on iphone or android. how can this be done? i have
seen links that cause to start of the native app, for example:

http://maps.google.com/maps?f=q&source=s_q&hl=en&geocode=&q=625+Nelson+Road,+Stanford,+CA+94305&sll=37.432558,-122.160627&sspn=0.012046,0.022724&ie=UTF8&hq=&hnear=Nelson+Rd,+Stanford,+Santa+Clara,+California+94305&z=16
