How to add two forms in a single page

Hi ,

Can someone please help me on how to make two forms in the login page. I am trying to create a federated login label for the openstack software. It already has a login and password label. I need to add one extra label that when clicked displays a list of IDP's. I have attached my design page along with this. I knidly request you to help me sorting out.
