[Google Maps API v2] form to indicate if one place (longitude, latitude) is userfriendly for disabled people, or not

Hi to all gurus !

first of all, I'm sorry for my poor english, I'm a frenchy guy !

I need a step by step help, or tutorial, for my project : 

within à google form, the user says if "yes" or "not" the place where he is, is disabled compliant.

for exemple, in front of one restaurant, if he can go in this restaurant, he selects "non" on my form

and send it.

I would like to have, the longitude, and latitude from where he send the form.

but I have not been able to achive the fusion tables.

Can I have some help for that ?
